Abstract: Background: Carcinoma of the cervix is the fourth most common cancer in women in the world.In 2018,an estimated 570000 women were diagnosed with cervical cancer worldwide and about 311000 women died from the disease. Bangladesh has the highest level of incidence and mortality rates due to cervical cancer among women due to lack of screening, high prevalence of risk factors and poor socio-economic condition. Methods: In this prospective observational study total 90 number of subjects were enrolled to compare the effectiveness of external beam radiation therapy by four field box technique versus two field technique in the treatment of locally advanced carcinoma of uterine cervix. Clinically and histologically diagnosed squamous cell carcinoma of uterine cervix patients were selected as study population. Results: In this study the highest number of patients were in between 46 to 55 years. Most of the subject used Oral contraceptive pill more than 5 years. Most of the patients were in stage IIB.Most of the patients were presented with postcoital bleeding, anemia, vaginal discharge , loss of appetite , Intermenstrual bleeding and pelvic pain. After completion of treatment follow-up per vaginal examination revealed that 73.33% subjects had complete response in four field group (Arm A). In two field group (Arm B), 57.78% had complete response. Common toxicities related to treatment were nausea, vomiting, diarrhea, skin reaction, anemia, leucopenia, bladder toxicity and rectal toxicity. Conclusion: From this study it can be concluded that four field box technique in EBRT was statistically effective in relieving symptoms and achieving complete response than two field technique in the treatment of locally advanced carcinoma of uterine cervix.

Abstract: Analysis was done to the clinical characteristics and outcome of children with infantile Fibrosarcoma (IFS) who were treated at Children’s Cancer Hospital Egypt (CCHE). A Retrospective chart review of all pathologically confirmed IFS patients treated at CCHE from July 2011 till December 2016. Detailed demographic, pathological, and clinical data of 27 IFS patients were collected, analyzed and correlated with treatment outcome and survival. We enrolled 27 patients with a median age 0.23 years (range 0.03-3.8), 54% were females. Extremities were the commonest primary site (59.3%), followed by head and neck (22.2%). The tumor size was >5cm in 18 patients, <5cm in six and undetermined in three. It was resectable without preoperative chemotherapy in three patients. Twenty one received preoperative VAC (Vincristine, Actinomycin-D and Cyclophosphamide) of those nine cases received second line Ifosphamide and Adriamycin to improve respectability. Overall survival of the whole group of patients was 82.3%. We need to increase sample size and to get facility for proper diagnosis by genetic analysis in such difficult cases for better pathological diagnosis

Factors Affecting Survival in Egyptian Patients with Advanced Vulval Cancer

Abstract: INTRODUCTION: Vulvar carcinoma accounts for about 4% of the female reproductive tract cancers and a total of 0.6% of all malignant cancers in females. PATIENTS AND METHODS: This retrospective study aimed to identify the possible risk factors which can impact survival in Egyptian patients with vulval cancer who received treatment at Ain Shams University department of clinical oncology and nuclear medicine in the period from 1-1-2007 till 1-1-2012. RESULTS: Our study included 64 patients [median age 63 years, median overall survival (OS) was 16.5 months]. The factors associated with favourable impact on OS were: Rural residence (17 versus 10 months for patients from urban residence, p=0.002), premenopausal status (19 versus 16 months in post menopausal patients, p<0.001), low grade histology (19 versus 10 months in patients with high grade, p<0.001) and negative resection margins (19 versus 10 months in case of positive margin, p<0.001). The factors associated with poor OS were: higher number of offspring (≥5 offspring, OS 10 versus 19 months if less than 5 offspring, p<0.001), patients presenting with ulcer rather than mass (11 versus 19 months, p<0.001 and those with bilateral disease (10 versus 17 months in unilateral disease, p=0.04), presence of ≥4 positive groin lymph nodes metastases (OS =16 months versus 17 months, p=0.047) tumor size ≥ 4 cm (10 versus 17 months in case of <4 cm, p=0.001) and depth of stromal invasion (17 versus 16 months in case of <1 cm and ≥ 1 cm respectively, p=0.015) CONCLUSION: Urbanization has been linked with more aggressive disease leading to decrease of OS. The age of onset of intercourse did not affect survival as was expected however multiple offspring which could be related to more frequent intercourse had an effect on survival. The offered treatment modalities did not show a superiority probably because all the patients presented in an advanced stage. The anatomical nature of this cancer might be the cause in delayed diagnosis which warrants health education projects for early detection. Further studies are required to assess the risk factors for development of vulval cancer in Egyptian patients.

Abstract: Acute lymphoblastic leukaemia is the most common malignancy in paediatric patients; its diagnosis is usually easy to establish as malignant lymphoblasts invade the bone marrow and peripheral blood. Patients with Acute lymphoblastic leukaemia (ALL) may initially present with pancytopenia and a hypoplastic or hyperplastic bone marrow. Fever is common at presentation, and despite neutropenia, sepsis is rarely seen. Other common clinical manifestations include fatigue, pallor, petechiae, bleeding, lymphadenopathy and hepatosplenomegaly. Presentation with primarily testicular disease is exceedingly rare in acute lymphoblastic leukemia. The unusual presentation of a primary left testicular involvement in ALL is being highlighted in this case report. We present an 11 year old boy who presented with fever, generalized body pain, abdominal discomfort as well as left testicular swelling. Bone marrow studies, Full blood count and peripheral blood film appearance were typical of acute lymphoblastic leukaemia. Ultrasound findings showed left scrotal enlargement with reduced and coarse parenchymal echotexture, while cytogenetic studies revealed positive Philadelphia chromosomes. There was good response to standard ALL therapy. Clinical remission with normal left testicular size was noted at the end of induction phase of therapy.
