Abstract: Objective: To explore the potential pathogenicity of epistaxis in routine diagnosis and treatment. Methods: The data of a patient with recurrent epistaxis and fever for more than 5 months were observed. Li Mou, a 21-year-old male patient complained epistaxis after digging his nose and was relieved after self-packing treatment before May. Due to the habit of blowing nose, digging his nose and staying up late in nights for playing games, he did not paid attention to his recurrent epistaxis 3 ~ 5 times a month. He had fever in January and his blood routine examination showed mild anemia. Autoimmune hemolytic anemia was considered after screening for blood system disorders. There was no remission after shock treatment with prednisone tablets, and the degree of anemia increased gradually. Frequent epistaxis did not improve after repeated nasal endoscopy, tamponade, cauterization and other treatment. He was admitted to hospital with severe anemia. After admission, the patient was examined for anemia, pale eyelid and conjunctiva, and it was fixed with oil gauze in both nasal cavities. A systolic murmur was heard in the apical region and the second auscultation area of aortic valve; the respiratory sounds of both lungs were rough, and there were no dry and wet rales. The abdomen was soft, without tenderness, rebound pain and muscle tension; there was no edema in both lower limbs. Two independent blood cultures were examined after admission, which showed defective hypoxic bacteria. Cardiac color Doppler ultrasound showed valve vegetations, perforation and reflux. Diagnosis of infective endocarditis caused by defective hypoxic bacteria was made after examination. Antibiotics combined with valve replacement surgery were performed for the treatment. Results: After one year follow-up, the symptoms of anemia and epistaxis were reveled. Conclusion: Routine diagnosis and treatment of epistaxis may be a potential risk factor for infective endocarditis; the presence of infective endocarditis and bacteremia may increase the frequency of epistaxis, resulting in multiple visits to hospital for treatment of epistaxis; in the diagnosis and treatment of autoimmune hemolytic anemia, it is necessary to screen infective endocarditis to rule out the adequate diagnosis.

Abstract: As one of the primary performances of the diesel engine, the special power can be promoted by increasing the speed or the indicated mean effective pressure. However, the diesel engine will not have enough time for fuel-air mixing and combustion under higher speed, so the excellent matching of injection-bowl is the main way to improve mixing and accelerate combustion. In order to achieve the rapid combustion of a high-speed diesel engine, the effects of piston bowl geometry, diameter-depth ratio, and nozzle arrangement on combustion and performance are calculated by using CFD numerical simulation. The results show that the mixing quality can be improved, the combustion duration can be shortened, and higher combustion efficiency and higher power can be obtained by using a double-layer diverging combustion chamber with a shallow basin structure in the upper and lower layers. With the increase of the diameter-depth ratio of the two-layer diverging combustion chamber, the maximum explosion pressure, the peak heat release rate and the turbulent kinetic energy increase, and the power rise is continuously improved. A new double-layer diverging combustion system matched with a double-layer stagger nozzle based on double-layer diverging combustion chamber the is proposed, the mixing in the cylinder is uniform, the combustion is accelerated and close to complete combustion, and the special power is 61.39kW/L.

Abstract: Intra-partum fever usually complicates the delivery process, and its occurrence is often considered synonymous with chorioamnionitis. This inevitably leads to the use of antibiotics for the affected mother. A review of the literature suggests that this approach is not always appropriate. Most cases of intra-partum fever are secondary to non-infectious factors. Both infectious and non-infectious maternal fevers are associated with transient adverse neonatal complications, while maternal fever at delivery is an important risk factor for long-term neonatal developmental outcomes, including encephalopathy, cerebral palsy, and neonatal death. Also prenatal antibiotic exposure increases the risk of developing allergic diseases in children such as asthma and eosinophilic esophagitis. Timing of intrapartum antibiotic administration is very challenging. More research is needed to explore the etiology of intra-partum fever, to discover ways to prevent and control maternal hypothermia, and the proper use of intra-partum antibiotics to reduce unnecessary antibiotic exposure to the fetus. Therefore, the purpose of this study was to review the prenatal and intra-partum factors associated with fever during labor and delivery and to find ways to prevent and treat them to reduce maternal and fetal complications. Effective measures were taken for management to reduce maternal and fetal complications.

Abstract: Based on a new pedestrian bridge project on Fuchun River in Hangzhou, this paper carries out special steel pipe pile cofferdam support design for its underwater pier cap during construction, analyses and studies several problems existing in the design of steel pipe pile cofferdam, carries out calculation and analysis according to two-dimensional and three-dimensional modeling and refers to the engineering experience of similar projects, and takes the following main solutions. Firstly, soil unloading measures are taken along the river bank. In order to reduce the earth pressure load on the shore side, the construction platform of the top-supported steel convenient bridge is built to reduce the horizontal construction load on the shore, and balance the water and soil sides by two measures to meet the safety requirements of the design of the steel pipe pile cofferdam, reduce the overall deformation and displacement of the steel pipe pile cofferdam, and during the construction of the underwater cap. The monitoring data of each foundation pit of steel pipe pile cofferdam are within the normal safety range, and the construction of underwater pier caps has been completed satisfactorily, which can provide design ideas and references for similar projects.

Abstract: Extracellular vesicles (EVs) play important roles in the diagnosis and treatment of diseases because of its lavish bioinformatic molecular contents. However, how to separate plentiful of extracellular vesicles in a short time with high consistency is still a problem. Here, we’d like to introduce an automated extracellular vesicles capturing machine named EVenrich (Extracellular Vesicles Enrichment) that helps realize capturing EVs of biosamples in a high-throughput manner. Incorporating EVenrich with EVtraps-a kind of biological magnetic beads capturing extracellular vesicles specifically, we were able to isolate EVs from up to 48 urine samples in 60 min, with similar quality and quantity and superior stability to manual capture. In clinical urine EVs analysis, over 14000 peptide fragments and 2000 proteins were detected by automated capture, which was similar to manual capture and significantly superior to ultracentrifugation. We further collected urine samples from 21 patients with negative prostate biopsy results and 21 patients with confirmed prostate cancer. Every three samples were mixed into a group to make mixed samples of 7 control groups and 7 disease groups. After exosome extraction and exosome proteomics / phosphorylated proteomics analysis, we found 211 overexpressed proteins and 187 overexpressed phosphorylated peptides corresponding to 40 phosphorylated proteins as potential biomarkers for prostate cancer. In addition, qRT-PCR results showed that the ratio of miR-125b to miR-145 could well distinguish prostate cancer patients from non-prostate cancer patients, suggesting that it could be used as a potential marker of prostate cancer.

Abstract: Objective: A systematic review of the effectiveness of cognitive behavioral therapy (CBT) and traditional non-drug methods in the treatment of neuropathic pain and anxiety. Method: PubMed, Embase, Cochrane, PsycINFO, Wanfang Database (WANFANG), China Knowledge Network (CNKI), VIP (VIP) and China Biomedical Literature Database (CBM) were electronically searched from the establishment of the database to March 2021 to collect randomized controlled trials (RCT) of the treatment of neuropathic pain. Two researchers independently carried out literature screening, data extraction and methodological quality evaluation, and cross-checked, and used RevMan5.3 software for Meta analysis. Results: A total of 15 RCTs with 1274 subjects were included, including 606 cases in the CBT group and 668 cases in the traditional treatment group. Meta-analysis results show that, compared with traditional non-drug treatments, CBT can significantly reduce neuropathic pain scores (MD=-0.55, 95%CI=-0.81~-0.29), and improve patients’ depression and anxiety (Baker Depression Scale), MD=-2.23, 95%CI=-3.84~-0.82; Hamilton Anxiety Scale, MD=-4.25, 95%CI=-6.20~-2.29). Conclusion: Current evidence shows that CBT has a good clinical effect on reducing neuropathic pain and improving patients' depression and anxiety. It can be used as a non-drug treatment for neuropathic pain in clinics, communities and families.

Abstract: The throughput of crude oil port is huge, and there are many toxic substances in crude oil, and the cumulative contact effect of toxic substances is obvious because the single operation time of the crude oil port operators is long. The purpose of this study is to investigate the hazard status of crude oil port and evaluate the occupational health and poisoning risk level, so as to provide crude oil operator for risk control. In order to evaluate the occupational health and poisoning risk of crude oil port operators, and to better protect operators, the exposure ratio was determined based on the detection of benzene and hydrogen sulfide concentrations, the main hazard factors of crude oil. Comprehensive analysis of benzene and hydrogen sulfide hazard characteristics, contact ratio, hazard control measures, exposure time and other factors of the exposure index, using the comprehensive index method to determine the exposure level, and finally determine the occupational health and poisoning risk of operators. To determine the occupational health and poisoning risks of workers. The assessment indicated that the risk rating for exposure to benzene and hydrogen sulfide was moderate, but there were weaknesses in hazard control measures. Once the protective effect of sanitary engineering is weakened or invalid, the workers will suffer serious occupational health and poisoning harm. Abstract: The process of sample punching on the stainless steel plate can calibrate the position and effectively improve the development of accurate positioning in the subsequent process. The processing technology of laser punching not only satisfies the efficient work of laser equipment, but also realizes the function of sample punching positioning, which is of great help to sheet metal processing. The laser punching process is affected by many process parameters which will result in slag inclusions in the punching socket after laser punching, in turn preventing the normal development of the subsequent process. In order to solve this problem, this paper sets out to study the influence of different process parameter combinations on the laser punching alignment quality by the orthogonal test method to formulate a scientific test plan for the process parameters involved, and the machining test of the punching alignment on the stainless steel plate with laser. The results show that the laser punching quality is affected by the marking time, laser power, auxiliary gas and air pressure under the condition that the distance from the nozzle remains unchanged, and the marking time has the greatest impact. In the meantime, this paper proposes the optimized combination of process parameters. The above results have instructive significance to the laser punching alignment process.

Abstract: In order to predict the effect of heavy ion irradiation more accurately, the ion distribution and stoping power of Au irradiated silicon (Si) and silicon carbide (SiC) under various energy conditions were calculated by using SRIM program, and compared with the experimental values. It was found that the projeced range and divergence predicted by SRIM were significantly smaller, and the stopping power was significantly larger by about 25%. Therefore, the method of reducing the target density parameters was tried to make corrections. It was found that the gold ion distribution became deeper after the density decreased, the stopping power became smaller, and the curve was closer to the experiment with an error of only ~5%. The density reduction coefficient is introduced since there is no definite conclusion on how much density decreases. Based on a large number of previous experimental data, SRIM was used to calculate the density reduction coefficient values under different energy conditions repeatedly, and found that there is a corresponding relationship between the values and energy values. Finally, the polynomial regression fitting method was used to obtain their functional expressions, which laid a foundation for the prediction of Si and SiC irradiated by heavy ions.

Abstract: Online drug sales are booming nowadays, and online sales of some prescription drugs are no longer prohibited by law, following over-the-counter ones. Because of its particularity, prescription drugs must be used according to doctor’s prescription to ensure their safety.In order to explore the illegal aspects in the current online prescription drug sales, the authors took three cases as the perspective and found out the following problems: Firstly, some online doctors do not hold valid certificates of medical practice; Secondly, doctors carry out first-diagnosis activities in violation of Internet Diagnosis and Treatment Management Measures; Thirdly, doctors do not follow the technical operation specifications, resulting in the diagnosis and treatment services are not compliant; Fourthly, doctors prescribe prescriptions in violation of the Prescription Management Measures. In addition, after the emergence of medical disputes, the responsibilities among online platforms, Internet hospitals and the physical medical institutions relied on are not clear, which is very detrimental to the rights protection of patients. The above problems seriously deviate from the safety principles that prescription drugs should follow. To study the current illegality of the current online prescription drug sales process, the author take the form of cases to carry out legal analysis. Furthermore, the authors hope that this analysis can attract the attention of the medicine, law, business and competent departments, so as to protect the basic human health rights of drug users right.

Abstract: In order to study the weak links of the China-Europe freight train logistics network, the countries and cities along the route are counted, and the China-Europe freight train container logistics network is constructed, and a node importance analysis method is designed by combining the node degree and betweenness of the network. In this paper, random attack and deliberate attack are used to study the changes of network characteristic values, such as the average degree of the network, the proportion of isolated nodes, the average path length, and the network efficiency. The research shows that under the deliberate attack, the node average degree, aggregation coefficient, network efficiency, and average path length of the China-Europe train network decrease rapidly with the increase of the attack ratio and decrease to 0. Under random attack, the average node degree, network efficiency, and average path length of the network show a slow decrease trend with the increase of the attack ratio, and the aggregation coefficient shows a trend of slow increase and then decrease. The isolated node is no longer connected to other nodes under the deliberate attack and random attack, so the attacked node is the isolated point, which increases the cardinality of the isolated node. The China Railway Express network is relatively strong under random attacks, and relatively vulnerable when it is deliberately attacked. It is found that some nodes in the China Railway Express play an important role in the connectivity of the network.

Abstract: Construction waste in Our country has a good recycling system, can carry out the production of various recycled building materials. In order to understand the availability of red brick in construction waste, the effect of the content of red brick in recycled aggregate on the physical properties and strength of recycled concrete is discussed to explore whether recycled concrete containing red brick is suitable for application in masonry blocks. The results show that there is no significant correlation between the content of red brick and the compressive strength of recycled concrete, while the slurry strength is positively correlated with the compressive strength of recycled concrete. The strength of recycled concrete can be improved by reducing the water-cement ratio. However, reducing the water-cement ratio will reduce the workability of concrete, which needs to be compensated by adding water-reducing agent. According to the experimental results of unit weight, water absorption, ultrasonic wave velocity and resistance properties of recycled concrete, the content of red brick can improve the porosity of recycled concrete and have a negative effect on durability. The overall experimental results are verified by the relevant specifications, and the recycled concrete containing red brick is suitable for class N block and meets the relevant requirements of the specifications.
