Transcranial Magnetic Stimulation for Post-Stroke Dyslexia 1 Case and Clinical Analysis

Abstract: Objective: To investigate the efficacy of repetitive transcranial magnetic stimulation therapy combined with speech training on the speech function of patients with post-stroke aphasia, understanding the mechanisms of cognitive neuropsychological recovery in patients with dyslexia. METHODS: One case of aphasia patients was collected and treated with repetitive transcranial magnetic stimulation and speech therapy, and the patients' Chinese language aphasia examination assessment and hydrogen protons nuclear magnetic resonance spectroscopy results were collected before and after treatment. Based on the combination of neurocognitive psychological model, the relationship between the change of speech function and the dual-channel model before and after patients' repetitive transcranial magnetic stimulation combined with speech therapy was investigated. RESULTS: After 2 months of treatment, the patient's reading function, conversation, comprehension, repetition, and naming function improved when compared with before, and hydrogen protons nuclear magnetic resonance spectroscopy suggested that the patient's bilateral cerebral hemispheres Cr/Ch, and NAA/Cr all increased after treatment compared with before. Conclusion: Transcranial magnetic stimulation therapy combined with speech training can promote the increase of brain metabolites and improve patients' reading function, providing a new therapeutic idea for the treatment of post-stroke dyslexia, Meanwhile, the combination of repetitive transcranial magnetic stimulation technique and hydrogen proton MRI spectroscopy can be used as a non-invasive treatment-examination combination to explore the mechanisms of recovery from dyslexia.

Abstract: Objective: To initially construct a comprehensive assessment system for upper extremity occupational competence based on the International Classification of Functioning, Disability and Health (ICF), and to provide a reference for the development of comprehensive intervention programs covering physiology, psychology, function and social participation for patients with upper extremity injuries. Methods: Taking the comprehensive ICF core set for vocational rehabilitation as the basic framework, using the Delphi expert correspondence method, after a two-round email-based survey with experts who are experience in clinical, vocational rehabilitation, rehabilitation, nursing. Combined with the statistical results and expert opinions, the index of the ICF upper limb vocational ability evaluation system was finally established. Results: The effective recovery rates of the two rounds of expert correspondence were 95% and 100%; the expert authority coefficients (Cr) were 0.807 and 0.805, and the Kendall coordination coefficients were 0.290 and 0.374 (P<0.001). The average importance value ranged from 3.52-4.48. The variation coefficients ranged from 0.103-0.222, The ICF upper limb vocational ability assessment system include 4 first-level indicators and 41 second-level indicators. Conclusion: The construction process of the ICF upper limb vocational ability assessment system is scientific and reliable, which promotes the clinical application and promotion of the brief version of the ICF core combination, and provides reference for the formulation of the ICF assessment combination for other diseases.

Abstract: Malignant tumors after surgical trauma are frequently presented with unavoidable complications, such as physical dysfunction, decreased motor endurance, bowel disorders and cognitive impairment, and other symptoms. Due to the presence of malignant tumors, patients suffer from not only physical discomfort but also the presence of psychological and mental stress. Trauma often leads to poor emotional control and cannot cooperate with treatment. Exercise can not only change the function of the limbs but also have a positive effect on emotions. This case reports a 28-year-old male who experienced a decline in cognitive function and activities of daily living after undergoing tumor resection and radiotherapy. A year later, he was hospitalized and completed a four-week activity of daily living and exercise rehabilitation training. What’s more, his mood was managed along with the reintegration therapy. As the patient's mood improved, training coordination increased, quality of life and daily living ability gradually improved, and he recovered well during the follow-up period. Given this single case, multi-modal exercise rehabilitation can maintain or improve functional performance and QOL domains even during heavy treatments. When the patient is undergoing exercise rehabilitation, he also needs emotional management, and emotional intervention is required for the patient. Therefore, exercise and the emotional management of patients with malignancy are of great importance.
